Any promotions or giveaways in addition to the :30 spots described above must be approved in advance. Please provide us with a promotional summary, on company letterhead, outlining each promotional element, the exposure Summerfest/Big Gig BBQ will receive from each element, the cash value of each element, and the total cash value associated with the promotion.

Yes, I have read and understand the information outlined above and agree to the terms and conditions outlined below.

You must provide proof of performance for your trade schedule in the form of a notarized affidavit, tear sheet and/or proof of performance report by no later than September 30, 2016. If proof of performance cannot be provided, you will no longer be considered for Summerfest trade.

Resale of tickets you receive as consideration for this trade is expressly prohibited. Use of tickets with any public promotion must be must be described above in a promotional summary and preapproved by Milwaukee World Festival, Inc. (“MWF”). Please report lost or stolen tickets immediately to MWF so that their bar codes may be cancelled.

Your failure to comply with the terms and conditions of this Ticket Trade Request Form constitutes a default under this agreement and may result in the immediate termination of this ticket trade and revocation of any unused tickets, without prior notice. Additionally, any misuse of tickets contrary to the terms of this agreement, or misrepresentation in your proof of performance, may prohibit your participation in ticket trades in the future. In the event you resell or engage in any fraudulent use of tickets, acquired pursuant to this Ticket Trade Request, MWF reserves the right to initiate any action necessary to protect its interests.
